Some issues

I'm having a hard time setting this game up -

If I use a lower resolution (640x400) I get letterboxing on the top/bottom. Not to mention it looks very fuzzy on my larger screen.

If I use a higher resolution (1280 or 1920) I get terrible mouse acceleration and it seems like the cursor snaps to buttons (moves slowly over them). Is there any way to disable this?

I have a wide screen (skareen?) running at 1440x900. you could force it - but really, it looks a lot better if you just play it in a window. go to settings from the program launcher and click the box "run window rather than full screen". I played it with 640x400.

Run winsetup.exe and play it with the resolution set to 320x200, but enable the '2x-nearest-neighbour' or '3x-nearest-neighbour' instead.

Note that you may need to run the game in windowed mode with the filters enabled. You may also need a fast system.
